A few SPOILERS below!
While Sam didn’t die, he is now just a little possessed. Right in the nick of time, Dean appears inside Sam’s mind as he is about to leave with Death. Dean begs Sam not to give up and that there is no him without Sam (if you didn’t squeal you have no heart!). Sam relented and said yes, he was gonna keep fighting because he can’t just abandon his brother! Dean morphs into Ezekiel and hugs the thoroughly surprised Sam.
Now in Sam’s body, Ezekiel tells Dean that Sam can’t know that he’s inside because if he’s ejected from the body, Sam will die. Dean understands and says that he’ll just have to not tell him until he’s fully recovered, but when will that be? Sam wakes up as himself inside the Impala as they are driving to some new location with no memory of being ready to die, meeting Death, or seeing Ezekiel.
I have to say that I loved Cas in this episode. The now human Cas is learning to live with the reality of feeling pain, needing to drink water, and doing laundry. Hence, the shirtless scene.
